Strategy 1

Use Dentin Saving, rather than Straight Line Access

Root canal procedures typically begin with establishing a glide path with a hand K-file. As the goal is to follow the canal, little if any tooth structure is removed. ** Please Review **

First generation NiTi files, and even heat treated files, required straight line access to ensure the safest procedure and prevent file breakage and separation. The rotary file therefore “brushed” along the canal wall to enlarge the coronal third to allow for the file to be relatively straight as it enters the canal. ** Please Review **

NiTi files that are designed for conservative endodontics utilize the next generation heat treatment process which produces a flexible file and retains sharpness. This allows the file to closely follow the canal anatomy without excessive dentin loss. Brushing is not needed and tooth structure is preserved.  ** Please Review **

Is it really possible to not need straight line access?

While it does give visual access to the canal, those practicing conservative endodontics find that “feel” is more important that visual queues of the canal. They believe that saving tooth structure is worth slightly altering their technique.

How does this technique differ from Minimally Invasive?

While we applaud the effort and those practicing Minimally Invasive procedures, in fact that would be our ultimate goal. However minimally invasive often brings with it the connotation of doing as little as possible to the tooth structure.  Initial access is kept to a bare minimum, and requires new tools and signifiant training. Conservative Endodontics on the other hand, is a philosophy that encourages tooth structure savings whenever possible using with only slight modifications to traditional techniques.  Conservative Endodtics states that any amount of structure saved is a net positive. Therefore general dentists, and endodontics alike can practice conservative endodontics.

Are special instruments required?

The key to conservative Endodontics is a rotary file that is designed for the technique. Flexible enough to not risk separation while sharp enough to work efficiently. A microscope is defiantly encouraged.
